Job summary

Open Sky Community Services is seeking a Respite Navigator to provide case management support to adults with complex mental health needs in the community. You will be the primary contact for providers across housing, finance, employment, and transportation, coordinating outreach and assessments to build individualized plans.

Qualifications include a High School Diploma/GED and experience in behavioral health; valid driver's license and ability to work independently are required.

Qualifications

• High School Diploma/GED or equivalent, required.
• Applicable experience in community-based or inpatient behavioral health, required.
• Experience with adults diagnosed with mental illness/substance use disorders and accessing local resources.
• Experience collaborating with hospitals, crisis programs, law enforcement, and DMH.

Responsibilities

• Collaborate with DMH funders regarding referrals.
• Establish and maintain relationships with funding, referral, regulatory and community agencies.
• Review and complete all required documentation.
• Acquire and maintain all required certifications.
