Job summary

Open Sky Community Services in Massachusetts is looking for a resilient Respite Navigator to provide case management support to adults with complex mental health needs in the Mobile Respite program.

You will engage with community providers, assess needs, coordinate housing, finances, employment, transportation, and develop person-centered treatment plans. This role operates within a trauma-informed, equity-focused team that values ongoing professional development and a supportive work culture.

Qualifications

• High School Diploma/GED or equivalent, required.
• Experience in community-based or inpatient behavioral health.
• Experience working with adults with mental illness/substance use disorders.
• Ability to navigate local resources and health systems.
• Valid driver's license and reliable transportation.

Responsibilities

• Collaborate with DMH funders regarding referrals.
• Establish and maintain relationships with funding, referral, regulatory and other community agencies.
• Review and complete required documentation.
• Acquire and maintain required certifications.
• Provide case management support, outreach, assessment, and person-centered treatment planning for Mobile Respite participants.
