Job summary

Open Sky Community Services is seeking a Respite Navigator to provide case management for adults with complex mental health needs in the community. You will connect clients to housing, employment, transportation, and social resources while coordinating with community providers.

The role requires experience in behavioral health, driving, and independent work abilities, with strong communication and collaboration with hospitals and DMH partners.

Qualifications

• High School Diploma or GED required.
• Experience in community-based or inpatient behavioral health.
• Experience with adults with mental illness/substance use disorders and navigating local resources.
• Experience collaborating with Hospitals, DMH, and other community partners.
• Valid driver’s license and reliable transportation required.
• Ability to work independently.

Responsibilities

• Serve as primary contact for community providers and coordinate referrals.
• Conduct outreach, engagement, and assessment for clients.
• Develop person-centered treatment plans for individuals in Mobile Respite program.
• Provide case management and connect clients to housing, employment, transportation, and other resources.
