Job summary

Open Sky Community Services is seeking a Respite Navigator in Massachusetts to support adults with complex mental health needs in the Mobile Respite program. You will connect individuals to housing, finances, employment, transportation, and community providers, acting as the primary contact for referrals and care planning.

Ideal candidates have case management experience within behavioral health, ability to work independently, and a valid driver’s license.

Qualifications

• High School Diploma or GED required.
• Experience in community-based or inpatient behavioral health preferred.
• Experience working with adults with mental health/substance use disorders.
• Experience working with community partners such as hospitals, DMH, crisis programs.

Responsibilities

• Provide case management support to adults in the Mobile Respite program.
• Coordinate connections to housing, finance, employment, transportation and services.
• Act as the primary contact for community providers; conduct outreach and treatment planning.
• Maintain documentation and acquire required certifications.
