Job summary

Terros Health seeks a Case Manager to join an interdisciplinary clinical team at the Oak Recovery Health Center in Phoenix, AZ. This full-time role supports individuals with Serious Mental Illness or General Mental Health diagnoses under Court Ordered Treatment.

Responsibilities include assessments, service planning, outreach, and collaboration with providers and families to ensure integrated care and positive outcomes.

Qualifications

• GED/HS diploma with 1.5 years behavioral health experience, or an associate degree in medical/behavioral health,
• or a bachelor's degree (not in behavioral health) with 1 year behavioral health experience,
• or a bachelor's degree in behavioral health, or a master's degree or higher in behavioral health.
• Proficient computer skills: Word, Excel, Outlook and EMR; NextGen preferred.
• Valid AZ Driver License; 4 years driving experience; must meet Terros Health driving policy.
• Valid AZ Fingerprint Clearance (Level 1) or apply within 7 working days of employment.
• Strong interpersonal and communication skills; DSM-V knowledge preferred; CCM strongly preferred.
• Must pass background check, TB test and other pre-employment screening.

Responsibilities

• Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals.
• Completes assessments, crisis plans and individualized service plans.
• Collaborates with the clinical team to engage, educate, and coordinate care with members, families, and providers.
• Conducts outreach and follow-up of services, including crisis intervention, missed appointments, and hospital discharges.
• Performs all case management functions including assessment and service planning; determines need for further evaluations.
• Promotes best practice outcomes in housing, employment and community tenure through ongoing education.
