Job summary

Terros Health is hiring a Case Manager to join our interdisciplinary clinical team at the 51st Ave Recovery Health Center in Phoenix, AZ. You will work with individuals diagnosed with SMI or general mental health issues under court-ordered treatment, providing case management and supportive services.

The role requires a High School Diploma or equivalent, with training provided; we offer full benefits, 401K, generous PTO, and opportunities for growth.

Qualifications

• GED/High school diploma with 1.5 years of behavioral health experience or
• An associates degree in medical or behavioral fields
• Bachelor's degree (in or not in behavioral health) with 1 year behavioral health experience
• Bachelor's degree in behavioral health or higher degree in behavioral health
• Master's degree or higher in behavioral health

Responsibilities

• Identify treatment and recovery goals and provide supportive services
• Complete assessments, crisis plans and individualized service plans
• Coordinate care with members, families and providers across teams
• Conduct outreach and follow-up including crisis interactions and missed appointments
• Perform all case management functions including assessments and service planning
• Pursue best practice outcomes for persons with mental illness and housing stability
