Job summary

Terros Health is seeking a Case Manager to join an interdisciplinary clinical team at the Oak Recovery Health Center in Phoenix. The role is full-time, 40 hours per week, Monday–Friday, 8:30 a.m.–5:30 p.m., located at 4451 E Oak St, Phoenix, AZ 85008.

Qualifications

• Completes assessments, crisis plans and individualized service plans.
• Works with the clinical team to coordinate care with members, families and providers.
• Engages in outreach and follow-up to ensure resources are available and in place.

Responsibilities

• Assist clients with identifying treatment and recovery goals and provide supportive services.
• Complete assessments, crisis plans and individualized service plans.
