Job summary

Terros Health is seeking a Case Manager for the Oak Recovery Health Center in Phoenix, AZ. This full-time role supports individuals with Serious Mental Illness or General Mental Health diagnoses under Court Ordered Treatment, coordinating care with a multidisciplinary team.

Position offers a comprehensive benefits package, 401(k), generous PTO/sick time, and a bilingual pay differential. 40 hours/week, Mon-Fri, daytime shift, on-site work at 4451 E Oak St, Phoenix, AZ 85008.

Qualifications

• High School Diploma or GED required.
• 1.5 years of behavioral health experience preferred.
• Formal education in behavioral health is advantageous (associates, bachelor’s, or master’s).
• Proficient in Microsoft Word, Excel, and Outlook; EMR experience a plus.

Responsibilities

• Assist clients with treatment and recovery goal setting and provide supportive services.
• Complete assessments, crisis plans, and individualized service plans.
• Coordinate care with clinical teams, families, providers, and community resources.
