Terros Health seeks a Case Manager to join the interdisciplinary clinical team at our 23rd Ave Recovery Health Center in Phoenix, AZ. This full-time role supports individuals with Serious Mental Illness or general mental health conditions under court-ordered treatment.

You will assess needs, coordinate care, and help implement the individualized service plan. We offer a comprehensive benefits package, including 401K, generous PTO, and an additional $1.50/hr SMI differential.

Qualifications

• GED/High school diploma with a minimum of 1.5 years of behavioral health experience, or
• An associatesdegree in the field of medical or behavioral, or
• A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, or
• A bachelor's degree in the field of behavioral, or
• A master's degree or higher in the field of behavioral health

Responsabilités

• Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals
• Completes assessments, crisis plans and individualizedservice plans
• Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualizedservice plan are implemented
• Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place.
• Performs all case management functions associated with caseload including completing the assessment and service planning processes; including identifying the need for furtheror specialty evaluations
• Pursues best practice outcomes for person with mental illness including continuing education, employment, independent housing and community tenure
