Job summary

Terros Health in Phoenix, AZ is seeking a Case Manager to join our interdisciplinary clinical team at Mitchell Recovery Health Center. This full-time position supports individuals with Serious Mental Illness or general mental health diagnoses under court-ordered treatment.

The role emphasizes assessments, service planning, and coordination of care with families, behavioral health, medical and dental providers, ensuring services are implemented as outlined in the plan.

Qualifications

• Must have GED/HS diploma or higher as listed in the qualifications.
• Associates, Bachelors, or masters in appropriate field are accepted with relevant experience.

Responsibilities

• Assists clients with identifying treatment and recovery goals and provides supportive services.
• Completes assessments, crisis plans and individualized service plans.
• Coordinates with clinical team, providers, family, and community resources to implement service plans.
• Performs outreach and follow-up including crisis interactions, missed appointments, and hospital discharges.
• Manages caseload including assessments and planning, pursuing continued education and housing outcomes.
