Job summary

Terros Health seeks an on-call Case Manager in Phoenix, AZ, to join our interdisciplinary clinical team. The role supports individuals with Serious Mental Illness or general mental health needs under Court Ordered Treatment, with training provided.

New grads welcome; bachelors in behavioral health is required. Duties include assessments, care planning, coordination across providers, and ongoing outreach to ensure services are delivered effectively.

Qualifications

• Bachelor’s degree in behavioral health required; no experience needed.
• On-call Employed role with training provided.
• Bilingual in Spanish a plus with differential pay.

Responsibilities

• Assist clients with identifying treatment and recovery goals and provide supportive services to help clients reach their goals
• Completes assessments, crisis plans and individualized service plans
• Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented
• Conducts outreach and follow-up of services including crisis intervention, missed appointments, and hospital discharges to ensure adequate resources are available and in place
• Performs all case management functions including assessment and service planning processes; identifying the need for further or specialty evaluations
• Pursues best practice outcomes for persons with mental illness including continuing education, employment, independent housing and community tenure
