Job summary

Terros Health seeks a passionate Case Manager to join our interdisciplinary clinical team in Phoenix, AZ. This full-time role (40 hours/week, M-F 8:30a-5:30p) serves individuals with Serious Mental Illness or General Mental Health diagnoses under Court Ordered Treatment at our South Mountain Recovery Health Center.

No experience is required with a Bachelors Degree in behavioral health; we will train you. New grads welcome.

Qualifications

• Experience in case management and coordinating care across providers.
• Ability to complete assessments and crisis plans with individualized service goals.
• Collaborate with clinical teams to educate, engage, and coordinate care with members and families.

Responsibilities

• Assist clients with identifying treatment and recovery goals and provide supportive services.
• Complete assessments, crisis plans and individualized service plans.
• Collaborate with clinical team to engage, educate, communicate and coordinate care with members, families, providers, and community resources.
• Conduct outreach and follow-up of services including crisis interactions, missed appointments, and hospital discharges.
