Job summary

Terros Health is seeking a Case Manager to join our interdisciplinary clinical team at Mitchell Recovery Health Center in Phoenix, AZ. The role is full-time (40 hours/week), Monday–Friday, 8:30am–5:30pm, located at 40 E. Mitchell Dr., Ste. #100, Phoenix, AZ 85012.

No prior experience is required, as a Bachelor's Degree in behavioral health is sufficient and training is provided. Benefits include a full package (401K, generous PTO/sick time, and health plans).

Qualifications

• Bachelor's degree in behavioral health required.
• No experience required for new grads; training provided.
• Ability to work with interdisciplinary clinical teams.

Responsibilities

• Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals.
• Completes assessments, crisis plans and individualized service plans.
• Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented.
• Conducts outreach and follow-up of services including crisis interaction, missed appointments, and hospital discharges to ensure resources are available.
• Performs all case management functions including assessment and service planning; determine need for additional evaluations.
• Pursues best practice outcomes for persons with mental illness including education, employment, housing and community tenure.
