Terros Health is seeking a Case Manager for the 51st Ave Recovery Health Center in Phoenix, AZ. The role involves coordinating care for individuals with serious mental illness or general mental health diagnoses under court-ordered treatment. Training provided; no experience required with a bachelor’s degree in behavioral health. Full-time, 40 hours/week (Mon-Fri, 8:30a-5:30p) at 4616 N.

51st Ave. #108. Eligible for $1.50/hr SMI differential, full benefits, PTO, 401K, and bilingual pay

Qualificações

• GED/High school diploma with 1.5 years of behavioral health experience, or associate degree in medical/behavioral field, or bachelor’s degree (non-behavioral) with 1 year of behavioral health experience, or a bachelor’s degree in behavioral health, or a master’s degree or higher in behavioral health.

Responsabilidades

• Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals.
• Completes assessments, crisis plans and individualized service plans.
• Works with the clinical team to coordinate care with members, families, and providers to ensure services are implemented.
• Conducts outreach and follow-up including crisis interventions, missed appointments, and hospital discharges.
