Job summary
Terros Health is hiring a Case Manager to join our interdisciplinary clinical team at the Mitchell Recovery Health Center in Phoenix, AZ. You will support individuals with Serious Mental Illness or General Mental Health diagnoses under Court Ordered Treatment, coordinating services and recovery goals.
The role is full-time (40 hours/week) with M-F hours and a comprehensive benefits package, including PTO, 401K, and health coverage.
Qualifications
- GED/High school diploma with BH experience (1.5+ years)
- Associates degree in medical/behavioral field or higher
- Bachelor's degree + BH experience or BH degree
- Master's degree in behavioral health preferred
Responsibilities
- Assist clients in identifying treatment and recovery goals.
- Complete assessments, crisis plans and service plans.
- Collaborate with clinical team to coordinate care with members, families and providers.
- Conduct outreach and follow-up to ensure resources are available.
- Perform case management tasks including assessments and planning.
