Job summary

Integral Care is seeking Continuity of Care Navigators to provide moderately complex coordination, initial assessments of clients with psychiatric and/or co-occurring disorders, and linkage to services to promote independence. This community-based role serves adolescents and adults in Travis County and involves discharge planning and resource coordination.

The position requires a Bachelor's degree in Human/Behavioral/Social Services (Master's preferred) and a valid driver’s license, with

Qualifications

• Bachelor's degree in Human/Behavioral/Social Services or related field; Master’s degree preferred.
• Minimum of 2 years of crisis experience.
• Valid driver's license and clean driving record.

Responsibilities

• Coordinate services and discharge planning to support independent functioning.
• Link clients to housing, employment, medical/dental/other essential services.
• Work under supervision with trauma-informed, recovery-based approach.
• Regular, predictable attendance and flexibility in schedule.
