Job summary

Adventist HealthCare seeks an experienced Inpatient Care Navigator for the Inpatient Behavioral Health Care Navigation Department. The role involves comprehensive patient assessments, care coordination, and discharge planning to ensure continuity of care during hospitalization.

Qualified candidates hold a Master’s degree in social work, counseling, psychology, or related field, with at least 2 years in behavioral health care management. Licensure (LMSW/LPC) is required; CCM/PMH-C is a plus.

Qualifications

• Master’s degree in social work, counseling, psychology, or a related field required.
• Minimum of 2 years of experience in behavioral health care management, case management, or a related field.
• Current licensure in a relevant field (e.g., LMSW, LPC) required; CCM/PMH-C certification is a plus.

Responsibilities

• Conduct comprehensive assessments of patients’ behavioral health needs upon admission.
• Develop, implement, and monitor individualized treatment plans with patients, families, and providers.
• Collaborate with psychiatrists, psychologists, social workers, nurses, and other care team members.
• Maintain accurate documentation of assessments and care plans per standards.
• Coordinate early discharge planning and connect patients to community resources.
• Advocate for patients, ensuring informed participation in treatment decisions.
• Contribute to quality improvement initiatives and crisis intervention as needed.
