Job summary

Mental Health Resource Center, Inc. is seeking a Care Coordinator to support individuals receiving outpatient medication management services.

You will collaborate with the APRN and Psychiatrist, monitor medication effects, coordinate care, and facilitate case and family conferences to promote independence and improve quality of life. The role emphasizes education for individuals, families, and caregivers on managing mental illness, with a focus on ensuring rights and safety through comprehensive

Qualifications

• Bachelor's degree in Social Work or related Human Services field from an accredited university or college.
• Experience in human services or mental health field preferred.
• Proficiency in MHRC EHR System within three months of employment; proficiency in Microsoft Office and email required.
• Must communicate and write English clearly at the college graduate level for records entries.
• Excellent customer service skills and ability to interact with diverse stakeholders.

Responsibilities

• Completes initial and ongoing assessments, including substance use, living situation, support systems, mental status, history, strengths/needs, and medical status.
• Monitors and supports mental stabilization through regular contact and assessment.
• Coordinates services outlined in treatment plans, including referrals to community resources, and evaluates effectiveness and satisfaction.
• Maintains regular contact during medication management appointments and via phone as needed.
• Advocates for individuals and ensures rights are upheld in accordance with state and federal regulations.
• Assesses risk factors, trauma symptoms, and signs of abuse, neglect, or abandonment; reports concerns per protocol.
