Job summary

Hill Country Mental Health and Developmental Disabilities Centers is hiring a Crisis Care Coordinator to provide crisis intervention and continuity of care across a county clinic and surrounding counties. The role includes developing recovery plans, coordinating with inpatient services, and delivering follow-up support to adults and children in crisis, with both in-person and virtual options.

Ideal candidates hold a bachelor’s degree in a relevant field and are eligible for QMHP-CS credentials,

Qualifications

• A bachelor’s degree from an accredited college or university in psychology, sociology, behavioral or human services.
• Must be eligible to be credentialed as a QMHP-CS.
• Must maintain Texas state Driver's License and auto insurance.
• Must become certified in PMAB, First Aid, and CPR within three months of employment.
• Preferred: experience with ANSA/CANS assessments or Texas Resilience and Recovery Model.

Responsibilities

• Assess individuals in crisis situations and develop individualized crisis treatment plans.
• Coordinate care with inpatient referrals as needed and provide crisis follow-up.
• Deliver Person Centered Recovery Plans in collaboration with individuals and families.
• Collaborate with local agencies, law enforcement, and EMS to support crisis services.
