Job summary

Community Reach Center seeks a Care Navigator to provide navigation and support services within its Aware Division. You will work with consumers to connect to behavioral health resources, using a case management approach and coordinating with various community supports.

The role involves documentation in the EHR (Credible), coordinating referrals, and partnering with a team to ensure timely and confidential service delivery. Requires a Bachelor’s degree and 1+ years in a related role.

Qualifications

• Requires Bachelor’s Degree in Health Management, Business Administration or another related field.
• 1+ years of experience in a similar management role is required.
• Strong communication, organization and time-management skills are essential.

Responsibilities

• Welcoming and registering new members, orienting them to programs and completing documentation in EHR.
• Provide navigation and case management support to consumers using integrated behavioral healthcare services.
• Enter new members into the EHR (Credible) and keep insurance information up to date; assist with paperwork.
• Enter, organize and update member data; ensure accurate data entry per CRC standards.
• Coordinate or co-facilitate groups, discussions, or one-on-one services and ensure timely data submission.
• Answer phones, direct visitors to needed resources, and act as liaison to connect consumers with community resources.
• Cross-train staff to improve service delivery and efficiency.
• Maintain organized greeters table and bulletin board with current information.
• Maintain confidentiality of consumer records and ensure data quality.
