Job summary

Mental Health Resource Center seeks a Care Coordinator in Florida to support uninsured and underinsured adults transitioning from higher levels of care to community-based services. The role focuses on assessment, care planning, and outreach to inpatient facilities, jails, and providers to ensure seamless support and recovery.

You will develop person-centered plans, coordinate across behavioral and physical health, housing, education, and employment, and advocate for access to essential services.

Qualifications

• Bachelor's degree in Social Work or related human services field.
• Minimum of one year of experience in human services or behavioral health.
• Proficiency in MHRC EHR System within three months of employment.
• Proficiency in Microsoft Office and email.
• Must have a valid Florida driver's license and be able to travel to satellite facilities.
• Strong communication and interpersonal skills.

Responsibilities

• Coordinates services across behavioral health, physical health, housing, education, employment, and other community systems to ensure comprehensive, person-centered care.
• Builds trust and rapport with individuals and their natural supports through outreach.
• Completes required screenings, assessments, and intake processes, including LOCUS.
• Develops, implements, and updates individualized, strengths-based care plans with individuals, families, providers, and supports.
• Monitors progress toward care plan goals and coordinates adjustments as needed.
• Advocates for access to community resources and services.
• Facilitates transitions and warm hand-offs between providers and community partners.
• Provides outreach to individuals referred from inpatient facilities, receiving facilities, jails, and other providers.
• Promotes recovery-oriented, culturally responsive, least-restrictive practices for community integration.
• Maintains timely, accurate documentation of assessments, care plans, contacts, and consents.
• Assesses safety risks and follows reporting procedures.
• Facilitates case and family conferences and maintains communication with the care team.
