Job summary

Brookdale in Eagan, MN is seeking a Clare Bridge Program Assistant to support dementia-friendly activities and programs. You will assist the Program Manager in delivering person-centered activities that enhance residents' quality of life and promote independence.

This role emphasizes teamwork, compassionate communication, and adherence to Brookdale standards. The position includes driving in some cases, coordinating events, and supporting volunteers, with a comprehensive benefits package

Qualifications

High school diploma or GED required; an associate degree is preferred.
Experience with older adults and dementia is preferred.
Must have valid driver's license and clean driving record.

Responsibilities

Assist in leading and implementing person-centered dementia programs.
Provide engaging, meaningful activities aligned with residents' routines and preferences.
Review and document residents' service plans and program outcomes.
Ensure programming complies with state and Brookdale standards.
Coordinate events and family/social activities within the community.
Communicate with residents and families about care and program benefits.
Support volunteer program including training and supervision.
