Job summary

The Landings of Rocky Mount Mills in Rocky Mount, NC seeks a compassionate Special Care Coordinator to lead Memory Care services, develop person-centered care plans, and guide staff training. This role partners with the Executive Director, healthcare teams, residents’ families, and care staff to ensure a safe, nurturing, and enriching environment for residents with cognitive impairments.

Qualifications

• Associate’s or Bachelor’s degree in Nursing, Healthcare Administration, or related field preferred.
• Active LPN or RN license preferred (depending on state requirements).
• Minimum of 2 years of experience in a senior living, assisted living, memory care, or healthcare setting.
• Previous supervisory or leadership experience in a healthcare environment is highly desirable.
• Strong leadership, communication, and interpersonal skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Knowledge of state regulations and compliance standards for assisted living and memory care.
• Proficiency in electronic health records (EHR) systems and basic computer skills.

Responsibilities

• Care Plan Development & Management: Develop, implement, and evaluate memory care programs.
• Staff Supervision & Training: Provide leadership, coaching, and dementia training to care staff.
• Resident & Family Engagement: Serve as the primary contact for residents and families.
• Regulatory Compliance & Quality Assurance: Ensure program complies with regulations and conduct quality audits.
• Collaboration: Work with nursing, physicians, therapists, and leadership to coordinate care.
