Job summary

Country Time Inn is seeking a compassionate Special Care Coordinator to lead Memory Care services, focusing on individualized care plans, staff development, and resident engagement. This role partners with the Executive Director and care teams to support a safe, enriching environment for residents with cognitive impairments.

The position emphasizes developing memory care programs, supervising staff, engaging families, and ensuring compliance with state standards to maintain high-quality care.

Qualifications

• Associate’s or Bachelor’s degree in Nursing, Healthcare Administration, or related field preferred.
• Active LPN or RN license preferred (depending on state requirements).
• Minimum of 2 years of experience in a senior living, assisted living, memory care, or healthcare setting.
• Previous supervisory or leadership experience in a healthcare environment is highly desirable.
• Knowledge of state regulations and compliance standards for assisted living and memory care.
• Proficiency in electronic health records (EHR) systems and basic computer skills.

Responsibilities

• Care Plan Development & Management: Develop, implement, and evaluate memory care programs; review and update care plans with families and teams.
• Staff Supervision & Training: Provide leadership and dementia training; manage staffing levels.
• Resident & Family Engagement: Be primary contact for residents and families; conduct meetings and share updates.
• Regulatory Compliance & Quality Assurance: Ensure program meets state regulations and conduct quality audits.
• Collaboration: Work with nursing, physicians, therapists, and leadership to coordinate services.
