Job summary

The Gardens of Pamlico is seeking a compassionate Special Care Coordinator to lead Memory Care services, focusing on individualized care plans, staff training, and resident engagement. You will partner with the Executive Director and care teams to support a safe, nurturing environment tailored to residents with cognitive impairments.

Responsibilities include developing care plans, supervising staff, engaging families, and ensuring compliance with state regulations and quality standards.

Qualifications

• Degree in Nursing or related field preferred.
• Active LPN/RN license preferred where required.
• 2+ years in senior living, memory care, or healthcare.
• Leadership and communication skills.
• Familiarity with EHR systems.

Responsibilities

• Care Plan Development & Management: develop and evaluate memory care programs.
• Staff Supervision & Training: lead staff with dementia training.
• Resident & Family Engagement: serve as primary care liaison.
• Regulatory Compliance & Quality Assurance: adhere to regulations and audits.
• Collaboration: work with nursing, physicians, and leadership.
