Job summary

Haywood Springs in North Carolina is seeking a compassionate Special Care Coordinator to oversee memory care services, develop individualized care plans, train staff, and elevate resident engagement and well-being.

This leadership role partners closely with the Executive Director, nursing team, families, and care staff to create a safe, nurturing, and enriching environment tailored to residents with cognitive impairments.

Qualifications

• 2+ years of experience in a senior living, assisted living, memory care, or healthcare setting
• RN or LPN license preferred depending on state requirements
• Previous supervisory or leadership experience in a healthcare environment is highly desirable
• Strong leadership, communication, and interpersonal skills
• Ability to manage multiple priorities in a fast-paced environment

Responsibilities

• Care Plan Development & Management: Develop, implement, and evaluate memory care programs and create person-centered care plans reflecting each resident’s history and evolving needs.
• Staff Supervision & Training: Provide leadership and dementia training to care staff and support scheduling.
• Resident & Family Engagement: Serve as the primary contact for residents and families and facilitate family meetings and care conferences.
• Regulatory Compliance & Quality Assurance: Ensure program compliance with state regulations and company policies; conduct quality audits.
• Collaboration: Work with nursing, physicians, therapists, and the Executive Director to coordinate care and support resident satisfaction.
