Job summary

The Landings of Rocky Mount Mills in Rocky Mount, NC seeks a compassionate Special Care Coordinator to lead Memory Care services, develop person-centered plans, and train staff to support residents with cognitive impairment.

You will partner with the Executive Director, families, and healthcare teams to maintain high standards of care and resident satisfaction in a caring, compliant environment.

Qualifications

• Must have an applicable healthcare degree and licensure.
• Experience with memory care knowledge is preferred.
• Strong leadership, communication, and regulatory awareness required.

Responsibilities

• Care Plan Development & Management: develop and evaluate memory care programs with family input.
• Staff Supervision & Training: provide dementia training and scheduling support.
• Resident & Family Engagement: serve as primary contact for care concerns and meetings.
• Regulatory Compliance & Quality Assurance: ensure programs meet regulations and best practices.
• Collaboration: work with nursing, physicians, and leadership to coordinate care.
