Job summary

Rich Square Manor and Villa in North Carolina seeks a compassionate Special Care Coordinator to lead Memory Care services, develop person-centered care plans, train staff, and engage residents and families.

You will collaborate with the Executive Director and clinical teams to ensure regulatory compliance, high-quality care, and a safe, enriching environment for residents with cognitive impairments.

Qualifications

• 2+ years of experience in a senior living, memory care, or healthcare setting.
• Supervisory or leadership experience in healthcare.
• Knowledge of state regulations and dementia care best practices.
• Proficiency in coordinating care plans with families and interdisciplinary teams.

Responsibilities

• Develop, implement, and evaluate memory care programs and care plans.
• Provide leadership, coaching, and dementia training to care staff.
• Serve as primary contact for residents and families regarding care.
• Ensure regulatory compliance and participate in quality improvement initiatives.
• Collaborate with nursing, physicians, therapists, and leadership.
