Job summary

Apploi is seeking a compassionate Nurse to join our Memory Care Unit in Chicago, IL. You will support a holistic, person-centered dementia care program and work with an interdisciplinary team to enhance residents' quality of life.

The role emphasizes ongoing dementia care education, supervision of aides, accurate documentation, family engagement, and adapting to residents' changing needs in a safe, supportive environment.

Responsibilities

• Commit to ongoing growth as a knowledgeable healthcare provider in dementia care.
• Commit to obtaining at least 12 hours of continual education annually, including orientation training.
• Assist in oversight of Memory Care Aides and CNAs, including supervision of ADL care and transportation.
• Become knowledgeable of each individual resident’s background, life history, disease, medical and behavioral care needs.
• Utilize communication devices for residents to enhance communication.
• Provide a therapeutic, person-centered memory care activity program to enhance quality of life.
• Encourage family involvement through volunteering and visitation.
• Provide a clean, safe, non-hazardous unit for residents and staff.
• Maintain an individualized symptom management tracking system and non-pharmacological interventions.
• Complete all required documentation and follow policies and regulations.
• Provide feedback to Memory Care Director for monitoring goals.
• Be flexible to adapt to residents’ changing needs.
• Provide emotional support to residents, families, and the inter-disciplinary team.
• Perform other duties as assigned.
