Job summary

All American Assisted Living is seeking an experienced LPN to join our Wellness team in providing compassionate, resident-centered care in an assisted living and memory care setting. You will administer medications, monitor residents, and document under RN direction to ensure safety and well-being.

This role requires teamwork, adherence to nursing standards, and clear communication with residents, families, and providers.

Qualifications

• Active LPN license in good standing
• Experience in assisted living or memory care preferred
• CPR/BLS certification
• Ability to follow physician orders and RN direction

Responsibilities

• Administer medications and treatments per orders and RN directions
• Monitor residents for changes in condition
• Obtain and document vital signs and health information
• Maintain accurate documentation of medications and nursing services
• Collaborate with RN and providers to adjust care plans
• Support admissions, transitions, and changes in condition
• Educate residents and families within scope
