The Alden Network Illinois Memory Care Unit seeks a Memory Care Nurse to help provide a holistic, person-centered program for residents with dementia. The Nurse will work to maintain the highest quality of life for each resident under our care.

Key duties include supervising Memory Care Aides and CNAs, delivering non-pharmacologic care, and documenting care plans in collaboration with the interdisciplinary team to meet regulatory standards.

Responsabilidades

• Commit to ongoing education in Alzheimer’s dementia and related diseases.
• Obtain at least 12 hours of continual education annually, plus the required 16 hours of orientation training and any dementia care trainings.
• Assist in oversight of Memory Care Aides and CNAs, including supervision of ADL care, transportation, and non-scheduled activities.
• Become knowledgeable of each resident’s background, disease, care needs, and preferences in detail.
• Utilize communication devices and language services to provide ample communication.
• Provide a therapeutic, person-centered memory care activity program and non-scheduled activities.
• Encourage family involvement through volunteering and visitation.
• Provide a clean, safe, non-hazardous unit for residents and staff.
• Assist in maintaining an individualized symptom management tracking system with non-pharmacological interventions.
• Complete all required documentation and follow Memory Care policies and procedures.
• Provide feedback to the Memory Care Director to monitor facility and program goals.
• Be flexible to adapt to changing resident needs.
• Provide emotional, caring support to residents, families, and the interdisciplinary team.
• Perform other duties as assigned.
