Job summary

Oakmont Management Group is seeking a Memory Care Director for Ivy Park of Milpitas to lead a Whole living dementia care program, partnering with residents and families to support independence and meaningful living.

The role oversees the Traditions memory care neighborhood, ensures safety, compliance with Title 22, coordinates staffing, and collaborates with culinary, health services, and activities to deliver individualized care plans.

Qualifications

• Two years of dementia care experience preferred.
• At least one year supervising and managing employees preferred.
• Must be eighteen (18) years of age.
• Resid encial Care Facility for the Elderly administrator license may be required.
• Able to obtain dementia care certifications (e.g., CDP, CAC, CAE, CDCM).
• Able to obtain and maintain CPR/First Aid certification.

Responsibilities

• Maintains a safe and secure environment for residents and staff.
• Ensures compliance with Title 22 regulations for memory care.
• Responsible for recruiting memory care staff; interviews, hires, onboards, trains, and retains.
• Coordinates staffing according to policy and resident needs.
• Communicates effectively with residents, families, and team members.
• Supports activity participation and dynamic daily programs.
• Collaborates with Culinary to meet nutrition needs.
• Assists in coordinating medical/dental/vision/podiatry care as needed.
• Partners with physicians, discharge planners, nurses, and agencies on care needs.
