Oakmont Management Group is seeking a Memory Care Director for Ivy Park of Milpitas. You will lead a Whole living dementia care program, partnering with residents, families, and team members to maximize independence and dignity.

This role oversees the Traditions memory care neighborhood and ensures care, activities, and dining align with personalized plans. You will recruit, train, and supervise staff, ensure regulatory compliance, and collaborate with health services to deliver comprehensive

Qualifications

• Two years experience with persons with dementia preferred.
• One year supervising and managing employees preferred.
• BA/BS in gerontology, psychology, sociology, or related field or 3+ years in social services.

Responsibilities

• Oversee the Traditions memory care neighborhood operations.
• Ensure dementia care, activities, and dining meet service plans and expectations.
• Collaborate with culinary, activities, and health services to customize resident care plans.
• Lead staffing, recruitment, training, and staff development.
• Partner with physicians, discharge planners, nurses, and state agencies as needed.
