Oakmont Management Group is seeking a Memory Care Director to lead the dementia care program at Ivy Park at San Tomas. This role focuses on promoting independence and dignity while coordinating care plans with culinary, activities, health services, and leadership teams.

The director will ensure safety, regulatory compliance, and staff recruitment across the memory care department, delivering personalized care plans that meet resident and family expectations.

Formación

• Must be eighteen (18) years of age.
• Prefer two (2) years of experience working with persons with dementia.
• Prefer one (1) year of experience supervising and managing employees.
• Preferred BA/S in Gerontology, psychology, sociology, or related field or at least 3 years working in social services, residential care, psychiatric facility, or related setting.
• Residential Care Facility for the Elderly administrators’ license may be required.
• Hold or able to obtain one or more certifications related to Dementia care; such as CDP, CAC, CAE, CDCM, or Montessori Dementia Care Professional.
• Able to obtain and maintain valid first aid and CPR certification.
• Able to obtain and maintain a valid Food Handler’s certification.

Responsabilidades

• Maintain a safe and secure environment for all team members, residents, and guests.
• Ensure compliance with state regulations concerning memory care (Title 22).
• Recruit, interview, hire, onboard, train, motivate, and retain memory care staff.
• Coordinate departmental schedule to ensure adequate staffing.
• Communicate effectively with residents, families, visitors, and staff.
• Support resident participation in activities and dining experiences.
• Collaborate with Culinary team to meet resident nutrition needs.
• Provide input to Health Services Director on assessments.
• Coordinate incidental medical and related care for residents.
• Partner with physicians, discharge planners, nurses, and state agencies on care needs.
