Responsibilities

• Welcome all new members according to Plan processes.
• Visit or contact assigned members at least monthly.
• Develop and maintain relationships with network providers and nursing home staff within the region.
• Sponsor or assist at least one group activity annually at each regional facility.
• Coordinate facility visits to ensure coverage and member needs are met.
• Document all activities as required.
• Follow up on items when members are dissatisfied and report to Plan.
• Ensure interactions with members comply with CMS regulations.
