American Health Plans is seeking a Member Advocate to support members within the Fort Worth region. This remote role requires travel to partner nursing homes and facilities to provide customer service, engage with families, and ensure member needs are met within CMS guidelines.

Responsibilities include welcoming new members, monthly outreach, relationship-building with providers, and documenting activities while maintaining compliance and scheduling visits with TruHealth staff.

Competenze

• Associate's degree preferred.
• Experience in a healthcare delivery setting with elderly patients preferred.
• State driver's license.

Mansioni

• Welcome all new members in accordance with established Plan processes.
• Visit or contact assigned members at least monthly.
• Develop and maintain relationships with key network providers, nursing home staff, and facilities within assigned region.
• Annually sponsor or assist a minimum of one group activity at each regional nursing facility.
• Contact TruHealth staff monthly to discuss needs and coordinate facility visits.
• Document all activities as required.
• Follow up on dissatisfied items and report to Plan.
• Ensure interactions comply with CMS regulations.
• Attend meetings.
• Other responsibilities as assigned.
