Job summary

American Health Partners seeks a customer-service focused leader to support local service areas. The Member Advocate will coordinate with nursing facilities, insurance producers, and care teams to ensure seamless enrollment and ongoing support for members and their families.

The role requires strong communication, organizational, and travel abilities within the assigned region, with emphasis on CMS regulations and high-quality member service.

Qualifications

• Experience in healthcare delivery settings, preferably with elderly or disability populations.
• Exceptional interpersonal and communication skills with the ability to work independently and as part of a team.
• Ability to coordinate with multiple stakeholders including nursing facilities, providers, and plans.
• Travel within assigned region with reliable transportation and valid driver's license.

Responsibilities

• Coordinate with Insurance Producer to ensure smooth enrollment and ongoing customer service.
• Follow up with members and families to answer questions and provide plan updates.
• Serve as frontline support with members, facilities, and care teams to meet member needs.
• Develop and maintain relationships with network providers within the region to support network adequacy.
• Meet with members or families at least monthly and report dissatisfaction items to the plan.
• Maintain reporting on member status, facility calendars, and issue tracking; ensure CMS compliance.
• Oversee services for a group of nursing facilities and ensure delivery across all members in those facilities.
