Job summary

American Health Plans is seeking a Member Advocate to support members across local service areas. This is a work-from-home role with substantial travel to partner nursing homes within the assigned region, enabling hands-on member engagement.

You will welcome new members, maintain monthly contact, collaborate with nursing home staff, and ensure compliance with CMS regulations while documenting activities and outcomes.

Qualifications

• Experience in a long-term care setting with elderly patients preferred
• Associate’s degree preferred
• State driver’s license required

Responsibilities

• Welcome all new members in accordance with Plan processes
• Visit or contact assigned members at least monthly
• Develop and maintain relationships with key network providers, nursing home staff, and facilities within assigned region
• Annually sponsor or assist a minimum of one group activity at each regional nursing facility
• Contact CMS staff or appointees monthly to ensure needs of member and Plan are met
• Document all activities as required
• Follow up on items members are dissatisfied with and report to Plan accordingly
• Ensure all interactions are compliant with CMS regulations
• Attend meetings as necessary
• Handle multiple priorities effectively
