Job summary

American Health Plans is seeking a Member Advocate to support local service areas and improve member experiences. You will welcome new members, visit facilities monthly, and build relationships with nursing home staff to ensure quality care and plan compliance.

You will work from a home workspace and travel to partner nursing homes within your region, coordinating with TruHealth staff and completing required training to maintain performance standards.

Qualifications

• Associate’s degree preferred.
• Experience in a healthcare delivery setting with elderly patients preferred.
• State driver’s license required.

Responsibilities

• Welcome all new members in accordance with Plan processes.
• Visit or contact assigned members at least monthly.
• Develop and maintain relationships with key network providers, nursing home staff, and facilities within assigned region.
• Annually sponsor or assist a minimum of one group activity at each regional nursing facility.
• Coordinate with TruHealth staff to discuss member needs and ensure coverage.
• Document all activities as required.
• Follow up on dissatisfied items and report to Plan accordingly.
• Ensure interactions comply with CMS regulations.
• Attend meetings as necessary.
• Other responsibilities as assigned.
