Job summary

American Health Plans in Topeka, KS is seeking a Member Advocate to provide exceptional, customer-focused support across local service areas. This is a work-from-home role with travel to partner nursing homes within a defined region and no fixed office space.

You will welcome new members, conduct monthly visits, cultivate relationships with network providers and nursing facility staff, sponsor group activities, document all activities, and ensure CMS compliance while coordinating with Plan

Qualifications

Experience in a long-term care setting preferred.
State driver’s license required.
Associate degree preferred.

Responsibilities

Welcome new members per Plan processes.
Visit or contact assigned members at least monthly.
Develop and maintain relationships with network providers, nursing home staff, and facilities within assigned region.
Annually sponsor or assist a minimum of one group activity at each regional nursing facility.
Contact TruHealth staff or appointees at least monthly to coordinate needs.
Document all activities as required.
Follow up on any member dissatisfaction and report to Plan accordingly.
Ensure all interactions comply with CMS regulations.
Attend meetings as necessary.
Other responsibilities as assigned.
