Responsibilities

• Welcome all new members per Plan processes.
• Visit or contact assigned members at least monthly.
• Develop relationships with network providers, nursing home staff, and facilities.
• Sponsor or assist at least one group activity annually at regional facilities.
• Coordinate facility visits to ensure coverage for the team.
• Document all activities as required.
• Address member dissatisfaction and report to Plan.
• Ensure interactions comply with CMS regulations.
• Attend meetings as necessary.
• Other responsibilities as assigned.
