Job summary

Qualifications

• Associate’s degree preferred.
• Experience in a healthcare delivery setting preferred.
• State driver’s license required.
• Excellent communication and listening skills.

Responsibilities

• Welcome all new members according to Plan processes.
• Visit or contact assigned members at least monthly.
• Develop and maintain relationships with network providers and nursing home staff within region.
• Annually sponsor or assist a minimum of one group activity at each regional nursing facility.
• Coordinate facility visits with the team to ensure coverage.
• Document all activities as required.
• Follow up on items of dissatisfaction and report to Plan.
• Ensure all interactions comply with CMS regulations.
