Job summary

Premier Home Health Providers Inc. is seeking a Medical Social Worker to lead a home-based social services team. The role focuses on policy development, care coordination, and mentoring social workers to deliver compassionate, high-quality home health services.

The position is per diem with a pay rate of $100–$120 per visit. You will collaborate with the Executive Director on budgeting, build community resource links, and ensure compliance with state and ACHC standards.

Qualifications

• MSW from an accredited program.
• Active California license as an LCSW.
• 3+ years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication and trust-building with patients, families, staff, and community.

Responsibilities

• Lead daily operations of the social services team, including policies, procedures, and care coordination with clinical staff.
• Recruit, onboard, and mentor social workers, providing supervision and professional growth.
• Oversee assessment, planning, and evaluation of social services care ensuring regulatory compliance.
• Partner with Executive Director on department budget and resource planning.
• Build knowledge of community resources and connect patients/referral sources to support.
• Plan in-service training and continuing education to maintain team skills.
• Contribute to quality improvement initiatives and identify ways to enhance patient care.
• Perform other duties as required for patient services.

• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
