Job summary

Premier Home Health Providers Inc. is seeking a per diem Medical Social Worker Leader in California to guide the social services team and mentor staff.

The role emphasizes clinical supervision, care coordination, and compliance with ACHC standards while supporting the organization’s mission of high-quality home health care. The position offers flexible per diem scheduling with a pathway to full-time based on performance.

Qualifications

• MSW degree required with CA licensure as LCSW.
• Minimum 3 years of management experience, preferably in home health.
• Strong knowledge of healthcare regulations and ACHC standards.

Responsibilities

• Lead daily operations of the social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision and supporting professional growth
• Oversee assessment, planning, and evaluation of social services care and ensure proper documentation
• Partner with the Executive Director on the department budget and resources
• Build knowledge of community resources and connect patients and referrals to support
• Plan in-service training and continuing education to keep skills sharp
• Contribute to quality improvement initiatives and identify ways to better serve patients
• Perform other duties as required for patient services

Please note that this role is per diem with a rate of $100 - $120 per visit.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
• A self-directed, organized, and flexible approach—you take initiative and work well with others
