Job summary

Premier Home Health Providers Inc. seeks a skilled Medical Social Worker (per diem) to lead our social services team, coordinate care with clinical staff, and mentor junior social workers in California.

The role requires an MSW, active California LCSW, and at least 3 years of management experience in home health, with strong knowledge of healthcare regulations and ACHC standards. This position offers a flexible schedule with potential conversion to full-time based on performance, plus benefits

Qualifications

• MSW from an accredited program.
• Active California LCSW license.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations, ACHC standards, and business operations.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth.
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards.
• Partner with the Executive Director on the annual department budget and resource planning.
• Build strong knowledge of community resources and connect patients and referral sources to the right support.
• Plan in-service training and continuing education to keep the team’s skills sharp.
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients.
• Perform other duties as required for patient services.
