Job summary

Premier Home Health Providers Inc. is seeking a Medical Social Worker to lead our social services in home-based care, guiding patients from facility to home and ensuring compassionate, high-quality service.

You will mentor a team of social workers, oversee assessments and care planning, and collaborate with clinical staff to meet state and ACHC standards. This per diem role offers flexibility and a path to full-time based on performance.

Qualifications

• Master of Social Work from an accredited program.
• Active California LCSW license required.
• Minimum of 3 years of management experience in home care.

Responsibilities

• Lead daily operations of the social services team and policy adherence.
• Recruit, onboard, and mentor social workers with clinical supervision.
• Oversee assessment, planning, and evaluation to meet ACHC standards.
• Partner with Executive Director on annual budget and resources.
• Build knowledge of community resources and connect patients to support.
• Plan in-service training and continuing education for the team.
• Contribute to quality improvement initiatives and patient service.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $100 - $120 per visit.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
• A self-directed, organized, and flexible approach—you take initiative and work well with others
