Job summary

Premier Home Health Providers Inc. seeks a Medical Social Worker (MSW) with management experience to lead our social services team in home health settings. You will supervise social workers, ensure care coordination with clinicians, and uphold ACHC standards to guide excellent patient care.

Responsibilities

• Lead daily operations of the social services team, including policies, procedures, and care coordination with clinical providers.
• Recruit, onboard, and mentor social workers with clinical supervision and growth support.
• Oversee assessment, planning, and evaluation of social services per state, federal, and ACHC standards.
• Partner with the Executive Director on annual budget and resource planning.
• Build knowledge of community resources and connect patients and referrals to support.
• Plan in-service training and continue education to keep team skills current.
• Contribute to quality improvement initiatives and assist patient care enhancements.

Please note that this role is per diem with a rate of $100 - $120 per visit.

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
