Job summary

Caringhand Home Health Solutions, Inc. is seeking a Medical Social Worker to shape home-based patient care, mentor a team of social workers, and set compassionate standards across our organization. This is a field-based, per diem role requiring travel to patients’ homes; not a remote/telehealth position.

Compensation is $100–$120 per visit. You will lead daily social services operations, recruit and supervise staff, ensure regulatory compliance, and drive quality improvement to enhance patient

Qualifications

• MSW degree from an accredited program.
• Active California LCSW license.
• Minimum 3 years of management experience, preferably in home health care.
• Knowledge of healthcare regulations and ACHC standards.

Responsibilities

• Lead daily operations of the social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers—providing clinical supervision and supporting professional growth.
• Oversee assessment, planning, and evaluation of social services care and ensure documentation meets state, federal, and ACHC standards.
• Partner with the Executive Director on annual department budget and resource planning.
• Build knowledge of community resources and connect patients and referrals to support.
• Plan in-service training and continuing education to maintain staff skills.
• Contribute to quality improvement initiatives and identify ways to better serve patients.
• Perform other duties as required for patient services.

• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
• A self-directed, organized, and flexible approach—you take initiative and work well with others
