Job summary

Caringhand Home Health Solutions, Inc. is seeking a Medical Social Worker to lead and mentor our social services team in the field. The role involves coordinating care, ensuring compliance with state, federal, and ACHC standards, and collaborating with providers across the organization.

Travel to patients’ homes is required for this per diem position. You will engage in budget planning, community resource development, and ongoing staff training to elevate the level of patient-centered care we

Qualifications

• Master of Social Work (MSW) from an accredited program.
• Active California license as an LCSW.
• Minimum 3 years of management experience in home care.
• Strong knowledge of healthcare regulations, ACHC standards, and business operations.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth.
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards.
• Partner with the Executive Director on the annual department budget and resource planning.
• Build strong knowledge of community resources and connect patients and referral sources to the right support.
• Plan in-service training and continuing education to keep the team’s skills sharp.
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients.
• Perform other duties as required for patient services.

This is a field-based, per diem position that requires travel to patients’ homes. Although listed as “remote,” this is not a work-from-home or telehealth role. Compensation is $100-$120 per visit.
