Qualifications

• Seasonal influenza vaccination required unless exempt for medical/religious reasons.

Responsibilities

• Register new patients and correct registration errors.
• Review and analyze medical records for completeness and proper sequencing.
• Verify that diagnosed conditions are substantiated by data and documented in sequence.
• Release PII to authorized parties and respond to inquiries with proper forms.
• Determine validity of requests and prepare PII forms and letters.
